Who Is Eligible to Become an Iris Clubhouse Member?
To be eligible for membership at the Iris Clubhouse an applicant must:

Be interested in attending the Clubhouse, as membership is voluntary

Have a diagnosed mental illness or be willing to seek a diagnosis

Not pose a threat to the community

Be 18 years of age or older
What Documents Will You Need to Apply?
To be considered for membership the following documents must be submitted:
1. An Iris Clubhouse Application
2. A completed psychiatric attestation form, from our website irisclubhouse.org, signed by a licensed LCSW, NPP, PSYD, MD, PA, LHMC, or LMSW (cosigned by supervising licensed professional)
3. We strongly encourage the inclusion of supporting psychiatric and psychosocial evaluations to further facilitate our membership review process.
4. Copies of all Health Insurance cards (please note however, membership is free of charge)
